How do I apply for a
Smile for a Lifetime scholarship?
To apply for one of our limited scholarships, you must fulfill and agree to all of the following criteria:
- Aged between 11-17, with a significant esthetic need for braces and a good dental hygiene routine. Your last dental hygiene check-up was in the last 6 months. You are fully committed to following and maintaining the treatment plan, agreeing to all morning appointments.
- You are a dedicated student, with good grades and a positive attitude, demonstrating a willingness to get involved in the community through volunteer service. You have two or more positive letters of recommendation from your teachers or community leaders.
- Your family income is no more than 200% of the federal poverty level.
- You are a resident of the Mid-Peninsula (East Redwood Shores, Redwood Shores, Menlo Park, Redwood City, San Carlos, San Mateo
